Responsibilities

• Evaluate patients' physical conditions, movement abilities, and medical history to develop personalized treatment plans
• Create and execute rehabilitation programs to improve functional deficits
• Perform manual therapy, therapeutic exercises, and use modalities such as ultrasound and electrical stimulation
• Provide patient, caregiver, and staff education on pain management techniques, fall risk safety, exercises, injury prevention, and pain management
• Track patient outcomes, document progress, and modify treatment plans as needed
• Collaborate with doctors, nurses, and other therapists to ensure comprehensive patient care

Requirements

• Graduate from an accredited Physical Therapy Program with a bachelor’s degree
• Current accredited MN Physical Therapist License
• Strong knowledge of state, federal, and local regulations pertaining to Home Health care
• Capable of performing the essential functions of the job, with or without reasonable accommodations
• Ability to communicate in English, both verbally and in writing, with sufficient communication skills to perform required tasks

Pay

$45 - $55 per hour depending on experience, plus up to a $7,500 bonus paid out over 18 months or tuition reimbursement.
