Responsibilities
- Conduct initial and ongoing evaluations of residents’ mobility, strength, balance, and endurance.
- Identify barriers to functional independence and design individualized physical therapy programs.
- Address challenges related to mobility, posture, gait, and injury or illness recovery.
- Provide interventions to help residents regain strength, prevent falls, and maintain independence.
- Support residents in daily functional tasks, recommending adaptive equipment as needed.
- Conduct exercises and mobility programs to enhance coordination, flexibility, and overall physical function.
- Assess living spaces to recommend modifications for safety and accessibility.
- Educate residents, families, and staff on therapy goals, safe mobility, and fall prevention.
- Collaborate with OTs, SLPs, nursing, and interdisciplinary teams to ensure holistic care.
- Maintain accurate records of assessments, treatments, and progress, ensuring compliance with healthcare regulations and community policies.
Requirements
- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
- Licensed as required by the State of Wisconsin to practice as a Physical Therapist.
