About the role

Greenfield Rehabilitation Agency is expanding into a new senior community in Janesville, Wisconsin. We are seeking a Full-Time or PRN Physical Therapist (PT) to join our team.

Responsibilities

• Perform comprehensive evaluations and develop individualized treatment plans
• Deliver high-quality, evidence-based physical therapy services
• Improve residents' mobility, balance, strength, and overall independence
• Supervise PTAs as appropriate
• Collaborate with an interdisciplinary team focused on exceptional outcomes
• Build strong relationships with residents and families
• Contribute to a positive, team-driven therapy culture
• Maintain accurate, compliant documentation

Requirements

• Licensed Physical Therapist (PT) in Wisconsin
• Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist

Qualifications

• Graduate of an accredited school with a minimum of a bachelor’s degree in Physical Therapy
