Responsibilities

• Performs evaluations and re-evaluations to assess patient conditions and progress.
• Establishes individualized treatment plans based on evaluation results and patient goals.
• Discharges patients by completing necessary documentation and providing follow-up recommendations.
• Documents patient care activities accurately and timely in the medical record.
• Charges for services rendered according to established billing procedures.
• Educates patients and caregivers about treatment plans, exercises, and home care instructions.
• Participates in practice changes resulting from performance improvement activities.
• Supports quality standards and initiatives set by the department.
• Maintains clinical competencies through ongoing education and training.
• Mets productivity standards defined by the department.
• Adapts to changes in patient census by adjusting work hours and assisting in other centers or programs as needed.

Requirements

• Entry level professional competency of Physical Therapy practices, theory, principles and applications as defined in competencies departmental policy
• Basic computer skills to include Outlook, Microsoft Office and Windows
• Bachelor's degree in Physical Therapy
• Five or more years of work experience in physical therapy
• Master's or Doctoral degree in Physical Therapy

Qualifications

• Physical Therapist license
• Basic Life Support - CPR Cert (BLS)

Skills

Knowledge, Skills, and Abilities: Entry level professional competency of Physical Therapy practices, theory, principles and applications as defined in competencies departmental policy [Required] Basic computer skills to include Outlook, Microsoft Office and Windows. [Preferred]
