PHP Step Down Coordinator
Peachford Hospital is a 246-bed, acute care psychiatric hospital located in the north-Atlanta Dunwoody neighborhood. The hospital features individual units for children, adolescents, adults, and seniors, and offers inpatient acute care, partial hospitalization, and intensive outpatient programs. About the Role
Due to continued growth in our outpatient services programs, we are searching for a Step-Down Coordinator to guide patients along their journey from inpatient to outpatient care. The PHP/IOP Step-Down Coordinator will meet with all eligible patients to discuss the benefits of Peachford's outpatient services as part of their continuum of care treatment.
Responsibilities
- Communicate patient eligibility for outpatient services to key members of senior leadership and associated departments.
- Routinely collect and enter data specific to outpatient eligibility into Midas for trending report purposes.
- Ensure patients have a thorough understanding of the benefits of outpatient services.
- Maintain a strong grasp of step-down processes and inpatient workflow, particularly as it pertains to case management and discharge planning.
Requirements
- Bachelor’s Degree (required); mental health/behavioral sciences field or equivalent preferred.
- Minimum of three (3) years’ experience in a healthcare setting or managed care company.
- Experience in patient assessment, treatment planning, utilization management, and/or case management.
