Pharmacy Technician Specialist – Medication History – Varied – La Jolla

Responsibilities

• Obtains accurate and complete medication histories for patients admitted to the acute care facility.
• Supports clinical pharmacists and providers by obtaining, verifying, and documenting patients' medication histories through direct patient interviews, caregiver communication, and review of external sources such as pharmacy records and prescription databases.
• Contributes to patient safety by minimizing medication discrepancies and supporting optimal pharmacotherapy decisions.

Requirements

• High School Diploma or equivalent
• California Pharmacy Technician License
• Certification by a national pharmacy technician certification board (CPhT) within 6 months of hire.
• ASHP Medication History Training Program within 1 year of hire
• Minimum 2 years' pharmacy technician experience

Qualifications

• Required Education/Experience/Specialized Skills:

• Prior experience obtaining medication histories from patients/caregivers
• Preferred Education/Experience/Specialized Skills/Certification:
  • CPhT and ASHP Medication History Certificate
  • 5 years' experience

Pay

Position Pay Range: $29.73-$41.71/hour
