Primary Duties and Responsibilities
- Conduct in-person or phone meetings with patients to update medication lists before provider appointments, ensuring accuracy and completeness.
- Ensure compliance with federal, state, DFCI, and Joint Commission policies, regulations, and standards.
- Create and update medication lists, document changes, and flag medications for removal to prevent harmful interactions.
- Communicate drug interactions or other changes with pharmacists and providers to ensure patient safety.
- Manage medication lists from external sources, maintaining comprehensive and accurate records.
- Utilize multiple systems to complete and document tasks, troubleshooting as necessary.
- Train other employees as needed.
- Communicate and escalate issues or problems.
- Provide cross coverage in hospital outpatient/specialty pharmacies as needed.
- Perform other duties as assigned.

Minimum Job Qualifications
- High school graduate or equivalent.
- Two years of prior work as a pharmacy technician in a retail or hospital setting required.
- Experience with oncology medications preferred.
License/Certification/Registration Required
- Massachusetts Board of Pharmacy Registration as a Pharmacy Technician required.
- National Pharmacy Technician Certification (PTCB) must be obtained within one year of hire date.
