Pharmacy Medication Reconciliation Technician
Responsibilities
- Conduct accurate medication reconciliation for new and existing patients.
- Verify and update patient medication lists at admission and discharge.
- Collaborate with pharmacists and healthcare teams to resolve med discrepancies.
- Maintain compliance with regulatory standards and institutional policies.
- Document reconciliation steps clearly in the EHR with audit-ready notes.
- Triage medication issues using defined escalation pathways.
- Demonstrate ownership of assigned patients and meet daily productivity targets.
- Achieve ongoing learning to advance clinical knowledge and technical skills.
- Aid in identifying potential drug interactions and contraindications.
- Support continuous process improvement and participate in metrics reviews.
- Prepare and present concise reports on reconciliation outcomes to supervisors.
- Maintain high visibility of work activity through regular status updates.
Requirements
- Pharmacy technician license or certification as required by state.
- Proven experience in medication reconciliation in clinical settings.
- Strong knowledge of EHR systems and accurate note documentation.
