Responsibilities
- Collaborate with pharmacists, providers, medical assistants, care coordinators, and health plan representatives.
- Absorb prescription renewals and refill authorizations.
- Coordinate medication access programs, copay assistance, and patient assistance resources.
- Identify patients requiring pharmacist intervention and facilitate warm handoffs.
- Maintain accurate records of outreach activities and intervention outcomes.
- Assist in developing adherence tracking reports and performance metrics.
- Support audits and quality assurance reviews.
- Monitor work queues and ensure timely follow-up on identified gaps.
- Review adherence reports and pharmacy claims data to identify patients at risk for non-adherence.
- Conduct outbound patient outreach via telephone, text messaging, mail, and other approved communication channels.
- Assess barriers to medication adherence including cost, transportation, side effects, forgetfulness, and access issues.
- Escalate clinical concerns and medication-related questions to the supervising pharmacist.
- Assist patients with medication refill coordination and prescription synchronization.
- Facilitate conversion to 90-day medication supplies when appropriate.
- Coordinate mail-order pharmacy enrollment and medication delivery services.
- Facilitate provider and pharmacy outreach with ability to communicate or triage medication requests.
- Document all patient interactions and interventions in the designated systems.
- Support adherence initiatives for: Statins, Diabetes medications, Renin-Angiotensin System (RAS) antagonists.
- Monitor adherence performance dashboards and patient gap reports.
- Participate in quality improvement projects designed to improve Star Ratings performance.
- Participate in weekly or monthly meetings with health plans to address adherence gaps and areas for improvement.
Qualifications
- Active Pharmacy Technician registration/licensure as required by state law.
- National certification preferred (CPhT).
- Minimum one year of pharmacy technician experience in retail, managed care, ambulatory care, or population health.
- Knowledge of medication adherence concepts and pharmacy workflows.
- Proficient with electronic health records and pharmacy management systems.
- Strong communication and customer service skills.
- Experience supporting Medicare Advantage or CMS Star Rating programs.
- Experience with population health, quality improvement, or care management initiatives.
- Familiarity with medication therapy management (MTM) programs.
- Bilingual Skills Preferred.
- Experience with Microsoft Excel functions.
- Valid CA Driver’s License and current vehicle insurance with clean driving record.
