About the role
The Medication Reconciliation Pharmacy Technician performs medication history assessments for new patients entering the Emergency Department or being admitted directly to the hospital. This includes obtaining medication information from patients, families, retail pharmacies, and physicians.
Responsibilities
- Enter or update medication histories into the electronic database
- Compile and verify patient medication lists
- Compare current prescriptions with medication lists
- Communicate discrepancies or questions to the patient's nurse, practitioner, and other healthcare providers
- Accurately document each medication identified (drug, dose, route, frequency, indication for PRN, and last dose taken-date/time)
- Verify patient allergy information and ensure complete documentation
- Communicate completed medication history to the physician, practitioner, and/or nursing team promptly
Requirements
- Working knowledge of common outpatient medication doses and forms preferred
- Necessary interpersonal and communication skills for interacting with patients, coworkers, and other health professionals
- Ability to prioritize work in a high volume work environment
- Ability to write legibly, speak, and read English proficiently
- Ability to operate computer, calculator, telephone
- Adaptable to changes in workload and schedule to meet patient needs
- Ability to set priorities and solve problems
Qualifications
- High School Diploma or GED required
- Successful completion of a certified pharmacy technician training program preferred
- Three or more years of pharmacy technician experience in a retail setting preferred
- Alabama Pharmacy Technician Registration Required
- Pharmacy Technician Certification (PTCB) or C-NHA-Certified Pharmacy Technician required to be completed within 6 months of hire
