About the role

The Medication Reconciliation Pharmacy Technician performs medication history assessments for new patients entering the Emergency Department or being admitted directly to the hospital under the supervision of a licensed pharmacist or healthcare professional.

Responsibilities

• Obtain medication history information from patients, families, retail pharmacies, or physicians' offices.
• Enter or update medication histories into the electronic database, ensuring accuracy and completeness.
• Compile and verify patient medication lists, compare with current prescriptions, and communicate discrepancies or questions to relevant healthcare providers.
• Accurately document each medication (including drug, dose, route, frequency, indication for PRN, and last dose taken-date/time).
• Verify patient allergy information and ensure complete documentation.
• Communicate completed medication histories to physicians, practitioners, and nursing teams promptly.

Requirements

• High School Diploma or GED.
• Successful completion of a certified pharmacy technician training program preferred.
• Three or more years of pharmacy technician experience in a retail setting preferred.

Qualifications

• Working knowledge of common outpatient medication doses and forms.
• Necessary interpersonal and communication skills to interact with patients, coworkers, and other health professionals.
• Ability to prioritize work in a high-volume work environment.
• Ability to write legibly, speak, and read English proficiently.
• Ability to operate computers, calculators, telephones.
• Adaptable to changes in workload and schedule to meet patient needs.
• Ability to set priorities and solve problems.
