Pharmacy Onboarding Intake Coordinator

We launched Mosaic Pharmacy Service in 2019 to offer pharmacy and chronic care management services for medically complex and vulnerable members, using data, advanced analytics, and pharmacy automation to transform the pharmacy experience for seniors while helping payers achieve quality improvement and cost savings.

About the role

Reporting to the Pharmacist-in-Charge, you will play a pivotal role in giving every new Mosaic patient a smooth, confident start with our pharmacy. You will help convert new referrals into fully onboarded, medication-ready patients quickly and accurately, reducing delays and errors at intake. As often the first voice a patient hears from Mosaic, you will help them feel informed, cared for, and ready to begin their new pharmacy service with confidence.

Responsibilities

• Serve as the primary point of contact for new patients via phone or video, guiding them through onboarding.
• Conduct comprehensive medication history interviews and reconcile prescriptions with new patients.
• Collaborate with prescribers to obtain new prescriptions.
• Address patient inquiries regarding insurance, costs, and coverage.
• Support claims processing and resolve third-party billing issues.
• Enter prescription details accurately into the pharmacy system to ensure precise, timely medication fulfillment.
• Document patient information thoroughly and accurately in the pharmacy management system.
• Educate patients on Mosaic's pharmacy services and benefits.
• Synchronize medication regimens and schedule each patient's initial medication shipment.
• Contact previous pharmacies to coordinate prescription transfers.

Requirements

• 3+ years of experience in a healthcare or pharmacy setting with direct patient interaction (home health, hospital, primary or specialty care, or pharmacy).
• Licensed pharmacy technician in good standing in the State of Virginia.
• Exceptional communication skills, including empathetic listening and clear verbal and written expression.
• High attention to detail, strong organizational skills, and adaptability in a fast-paced, evolving environment.
• Proficiency with pharmacy software and general computer applications.
• Available to work an 8.5-hour shift, Monday through Friday, between 9:00 a.m. and 6:00 p.m. ET (exact shift confirmed at hire); occasional overtime may be required based on patient needs.
• Must be authorized to work in the U.S. without company-sponsored visa sponsorship.

Preferred Qualifications

• 1-2 years of experience in a call center environment.
• Experience collecting medication histories.

All offers of employment are contingent upon the successful completion of a comprehensive background check and pre-employment drug screening.

Pay

The expected salary range for this position is $26/hr to $28/hr. Actual compensation within the range is determined by factors including experience, skills, and internal equity.

Schedule

Full-time, Monday through Friday. Available to work an 8.5-hour shift between 9:00 a.m. and 6:00 p.m. ET (exact shift confirmed at hire). Occasional overtime may be required based on patient needs.
