About the role

The Pharmacy Infusion Navigator-RN serves as an expert clinical consultant to hospital staff and a clinical educator and advocate for patients. The role focuses on raising awareness about the benefits of home infusion and optimizing the transition of care for patients referred to Norton Home Infusion.

Responsibilities

• Educate patients, physicians, case managers, and all customers regarding disease states, infusion therapy, transitional care management, and other Norton Home Infusion programs and services.
• Facilitate timely placement of discharge patients requiring home infusion or enteral services by gathering preadmission data and collaborating with the hospital discharge team.
• Perform initial patient set-up/training and provide ongoing patient/caregiver education.
• Interface with healthcare professionals and hospital staff to strengthen partnerships and grow business through referrals.
• Work independently and manage tasks effectively in a fast-paced environment.
• Collaborate as a team player with a positive attitude in a cross-functional, matrixed environment.

Requirements

• Minimum of three (3) years of clinical (RN) nursing experience.
• Bachelor’s Degree.
• Active Compact/Multistate Registered Nurse (RN) license.

Qualifications

• Minimum of three (3) years working in a healthcare or pharmacy organization (desired).
• Excellent clinical, interpersonal, and communication skills.
