About the role
This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment.
Responsibilities
- Evaluate the appropriateness of drug therapy based on patient specific factors
- Dispense and provide medications
- Provide pharmacokinetic and anticoagulation consultations
- Review and interpret medication orders
- Ensure all medication orders are entered into the electronic patient medical record
- Complete patient chart reviews, evaluate pertinent laboratory data, drug-drug and drug-nutrient interactions, monitor for adverse drug effects, and screen for allergies
- Communicate findings with prescribers and provide appropriate alternatives to current treatment plans as needed
- Obtain medication histories (Med Recon) from patients and summarize significant findings in the medical record and/or to the provider as appropriate
- Monitor and report drug errors, adverse drug reactions, allergies, and patient compliance issues
- Document findings per facility procedures
- Aid in administrative paperwork as requested by supervisor(s)
- Aid in training new staff who will be assigned to perform WHEN hour duties
- Ensure staff continue to follow proper processes and procedures
- Assist with oversight of daily operations and facilitate workflow to ensure operations run smoothly
- Assess workload volume throughout the evening shift and identify backlogs to make staffing adjustments as needed to complete assigned tasks and meet goals
- Serve as an authority on drug usage, interactions, over-dosages, and compliance to medical staff, nursing staff, technicians, and students
- Research drug information using appropriate references
- Maintain a current knowledge of therapeutics and disease management
