Pharmacist - Clinical

Responsibilities

• Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals.
• Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions.
• Deliver medication therapy management with a focus on antibiotic stewardship and infectious disease protocols.
• Ensure pharmaceutical care programs are appropriately integrated throughout the institution.
• Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to antimicrobial therapy management.
• Conduct Medication Use Evaluations (MUEs) with a focus on antibiotics/ID, and recommend necessary changes to pharmacotherapy practice.
• Serve as a resource in antibiotic stewardship initiatives, ensuring best practices and compliance with institutional standards.

Qualifications

• Education: PharmD or BS in Pharmacy
• Experience: Pharmacy practice residency or equivalent hospital experience (3 postgraduate years)
• Preferred: Infectious Disease training and/or experience in antibiotic stewardship
• Certifications: Current Pharmacy license in Massachusetts, Board Certification in Pharmacotherapy (BCPS) or actively working toward certification
