Pharmacist - Clinical

About the role

The Clinical Pharmacist provides exceptional care for patients with specialized pharmacy requirements. Responsibilities include gathering and assessing clinical data, creating and supervising care plans, communicating with prescribers, reporting outcomes, making therapeutic suggestions, and overseeing therapy care specialists in compliance with state, federal, and accrediting body regulations, as well as Anovo Specialty Pharmacy policies and procedures.

Responsibilities

• Oversee dispensing, pharmacy operations, and pharmacy personnel (technicians and other associates).
• Comply with FDA mandates and manufacturer restrictions.
• Evaluate medication orders for appropriateness of drug, dosage, route of administration, potential drug interactions, high-risk patient considerations, warnings/precautions, potential adverse reactions, and stability.
• Suggest therapeutic interventions where appropriate and ensure availability for consultation to residents, prescribers, nursing staff, and designated healthcare representatives.
• Maintain patient/resident medication profiles including current and past medications, medical history, clinical/laboratory data, and relevant clinical notes.
• Procure and maintain required documentation in accordance with federal, state, and AnovoRx policy.
• Dispense pharmaceuticals including biotech drugs and related items for patients with rare diseases.
• Provide drug information, counsel patients, adhere to state/federal licensure requirements, and maintain required paperwork.
• Participate in Continuing Quality/Improvement programs and ensure compliance with all standards.
• When AnovoRx has written agreements with manufacturers for pharmacovigilance services:
  • Participate in required training.
  • Receive reporters’ calls concerning adverse events, product complaints, and medical inquiries.
  • Complete submissions to the manufacturer and designated partners.
  • Assist with follow-up as required.
• Maintain patient confidentiality and regulatory requirements, including federal, state, local, and accrediting organization standards.
• Wear identification while on duty and represent the organization professionally.
• Attend work regularly and reliably.
• Perform other work-related duties as assigned by supervisor/manager.

Requirements

• Bachelor’s degree in Pharmacy or Graduate degree from an accredited school of pharmacy.
• 1+ years of pharmacy experience preferred.
• Current unrestricted license or registration/certification in the required state(s) of practice, as required by law.
• Willing and able to work any dedicated 8-hour shift between 7:00 AM and 7:00 PM.
• Willing and able to work onsite on regularly scheduled shift Monday–Friday, if applicable.
• Willing and able to cover on-call rotation as needed.
• Professional presentation and clear communication, both verbally and in writing.
• Comfortable with repetitive motions in answering phones and using a computer keyboard.
• Adept at handling sensitive and confidential situations.
