About the role
We are seeking an AMS Pharmacist to lead anticoagulation management, ensuring patient safety and optimal outcomes. In this clinical role, you will manage warfarin therapy, oversee peri-procedural bridging, and provide high-level medication therapy management and patient counseling.
Responsibilities
- Manage warfarin therapy and oversee peri-procedural bridging
- Provide high-level medication therapy management and patient counseling
- Coordinate care transitions and hospital discharges
- Triage INR reviews and clinical assessments
- Supervise pharmacy technicians
- Develop clinical guidelines
- Ensure regulatory compliance and clinical safety
Qualifications
- Doctor of Pharmacy (PharmD) or BSc in Pharmacy with 3+ years of experience
- Licensure as a Registered Pharmacist
- Anticoagulation management experience (preferred)
