Job Description
The role of Pharmacist at Waltz Health involves providing expert medication management, clinical support, and patient education for individuals with complex medical conditions. Key responsibilities include:
- Collaborating with healthcare providers, insurance companies, and other members of the healthcare team to ensure timely access to prescribed therapies.
- Assisting in managing prior authorizations, insurance verifications, and formulary options to facilitate patient access to medications.
- Reviewing and managing prescription data entry, medication interactions, contraindications, and other safety considerations related to specialty medications.
- Collaborating with Patient Care Coordinators and other pharmacy team members to ensure seamless service and support for patients.
- Assisting patients in understanding the therapeutic goals of their treatment, helping them manage their conditions and improve quality of life.
- Offering support and resources for managing financial barriers, insurance, and access to medication assistance programs.
- Documenting clinical interventions, medication adjustments, and follow-up actions to ensure continuous care and adherence to therapy within the pharmacy management system.
- Maintaining compliance with regulatory requirements, such as HIPAA, for patient confidentiality and documentation standards.
- Participating in quality assurance and improvement initiatives to enhance the efficiency and effectiveness of patient care.
- Monitoring medication therapy outcomes and patient satisfaction, providing feedback to improve services and reduce potential risks.
- Staying current with the latest trends, therapies, and guidelines in the specialty pharmacy field.
- Engaging in continuous learning and professional development through attending conferences, seminars, and other education opportunities.
- Mentoring and supporting other pharmacy team members in their clinical education and career growth.
Qualifications
- A pharmacy degree from an accredited School of Pharmacy.
- Active Illinois pharmacist license in good standing.
- Minimum of 3 years of experience practicing pharmacy, specialty pharmacy or a related field is preferred.
- Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy school.
- Additional multi-state pharmacist licensure.
- Specialty Pharmacy Certification from the National Association of Specialty Pharmacy (NASP).
- Board of Pharmacy Specialties (BPS) certification (i.e. Pharmacotherapy, Oncology Pharmacy).
- Experience in patient counseling, medication therapy management, and prior authorization processes.
- Knowledge of insurance formularies, specialty medications, and drug therapies used in the treatment of chronic or complex diseases.
Pay Range
$100k - $149k annual salary
