About the role

This onsite Clinical Pharmacist role focuses on providing specialized patient care in a non-retail pharmacy setting. You will provide clinical oversight for an assigned patient care team, deliver high-touch counseling to patients by phone, and ensure all prescriptions are dispensed safely and in full compliance with state, federal, and board of pharmacy regulations. The position is based in Shepherdsville, Kentucky, and requires living within approximately one hour of the facility.

Responsibilities

  • Conduct comprehensive clinical reviews of pharmacotherapy for each patient, including drug utilization, adherence counseling, drug interaction screening, side effect profiling, and drug administration support to ensure treatment plans align with evidence-based recommendations.
  • Document the results of pharmacotherapy reviews in the patient chart so that nursing staff performing follow-up assessments are fully informed and educated on the care plan.
  • Counsel patients on new drug therapies in accordance with clinical management models, ensuring they understand the specifics of their planned treatment and feel supported throughout their therapy.
  • Complete required REMS training and strictly follow program standard operating procedures to maintain compliance and patient safety.
  • Cookordinate patient care with referring clinicians, addressing social, psychological, and physical needs that may require customized pharmacy services such as pre-filled syringes and color-coded labeling, and document these services clearly for fulfillment and distribution teams.
  • Serve as a key liaison between patients, caregivers, home health agency nurses, hospice nurses, and prescribing clinicians to ensure seamless communication and outstanding service for all stakeholders.
  • Provide subcutaneous administration teaching and support for patients and caregivers, and coordinate appropriate nursing services with prescribing clinicians when needed.
  • Provide on-call coverage as directed by the Pharmacist-in-Charge to address patient needs outside of standard operational hours.
  • Process prescriptions accurately and in a timely manner to support downstream workflow efficiency and ensure patients receive medications as prescribed.
  • Verify discontinuation of service for each patient by assigning the appropriate therapy outcome, deactivating the patient’s chart, prescriptions, and software profile, and notifying the Patient Support group to discontinue future refills.
  • Implement clinical management models for specific products and disease states to standardize care and improve patient outcomes.
  • Act as the primary intake contact for all new team referrals initiated by telephone, ensuring accurate and thorough information gathering.
  • Demonstrate a strong commitment to exceptional customer service in line with internal policies and foster positive relationships with patients and clinicians.
  • Adhere strictly to company policies related to all government-funded payers, including Medicaid and Medicare, maintaining compliance with applicable regulations.
  • Dispense all medications pursuant to a valid physician’s order and in accordance with all state, federal, and board of pharmacy laws.
  • Maintain full compliance with company policies, particularly those governing pharmacy operations and quality standards.
  • Adhere to federal and state laws regarding the purchase, handling, and dispensing of controlled substances.
  • Verify the accuracy of prescription order entry early in the workflow, correcting any errors or discrepancies to improve efficiency for subsequent teams.
  • Perform accuracy and quality assurance checks on final dispensed prescription products to ensure they meet all safety and quality requirements.
  • Assist pharmacy technicians in adjudicating pharmacy claims to ensure accurate payment is communicated and collected.
  • Monitor the management of hazardous waste materials within the pharmacy to ensure safe handling and regulatory compliance.
  • Understand and supervise the aseptic technique used by pharmacy personnel, promoting high standards of sterile compounding and quality control.
  • Utilize patient education materials effectively to reinforce counseling and support patients in managing their therapies.
  • Perform intake of adverse events and product quality complaints and ensure appropriate reporting in accordance with regulatory and contractual obligations.
  • Meet or exceed continuing education requirements as mandated by board of pharmacy laws and maintain current professional knowledge.
  • Support the professional development goals of staff within the assigned patient care team, encouraging continuous learning and skill growth.
  • Attend professional conferences, collaborate with peers, and share key learnings and topics with the team to enhance collective knowledge.
  • Provide excellent oral and written communication in all interactions, including documentation, patient counseling, and collaboration with clinicians.
  • Work extensively by phone, counseling patients for 7 to 8 hours per day, and maintain a patient-centered, empathetic approach to care.

Requirements

  • Must live within approximately one hour driving distance of the Shepherdsville, Kentucky facility and be able to work onsite.
  • Must be licensed to practice pharmacy in Texas and hold a current, unrestricted license in good standing; licensure in North Carolina or Kentucky is also acceptable if in good standing.
  • Must have at least 2 to 3 years of direct patient counseling experience, preferably in a clinical pharmacy setting.
  • Must have 3 to 5 or more years of post-graduate experience in pharmacy operations, clinical oversight, and patient care.
  • Graduate of an accredited school of pharmacy with the educational credentials required for pharmacist licensure.
  • Ability to apply for and obtain additional state pharmacist licenses as needed to support patient care.
  • Demonstrated enjoyment of counseling patients and comfort being on the phone for 7 to 8 hours per day.
  • Strong clinical skills in pharmacotherapy review, including drug utilization assessment, adherence counseling, drug interaction screening, side effect profiling, and drug administration support.
  • Knowledge of and adherence to state, federal, and board of pharmacy laws, including regulations governing controlled substances and government-funded payers such as Medicaid and Medicare.

  • Experience coordinating care with clinicians, nurses, caregivers, and other healthcare professionals to support comprehensive patient care.
  • Strong oral and written communication skills for patient counseling, documentation, and collaboration with interdisciplinary teams.
  • Working knowledge of computers and excellent word processing skills for documentation and prescription processing.
  • Working knowledge of sterile technique and quality control practices in a pharmacy setting.
  • Commitment to meeting or exceeding continuing education requirements as defined by board of pharmacy regulations.
  • Ability to provide on-call coverage as directed to meet patient needs outside regular operating hours.
  • Strong customer service orientation and ability to build and maintain positive relationships with patients and clinicians.

Qualifications

  • Board Certified Oncology Pharmacist (BCOP) certification is highly valued; willingness to become BCOP certified when eligible is preferred.
  • Experience working in a non-retail, clinical or specialty pharmacy environment is advantageous.
  • Familiarity with REMS programs and related standard operating procedures is beneficial.
  • Experience coordinating customized pharmacy services such as pre-filled syringes and color-coded labeling for medication identification.
  • Experience in managing hazardous waste materials and supervising aseptic technique in pharmacy operations.
  • Experience overseeing pharmacy technicians and supporting their development and performance.
  • Demonstrated commitment to continuous professional development through conferences, peer collaboration, and self-directed learning.

Pay and Benefits

The pay range for this position is $60.00 - $60.00/hr.

Work Environment

This is an onsite Clinical Pharmacist position based in Shepherdsville, Kentucky, within approximately one hour driving distance of the facility. The role is in a non-retail pharmacy environment focused on specialty and clinical patient care rather than walk-in customers. Training is conducted Monday through Friday from 9:00 a.m. to 6:00 p.m. CST, after which the regular shift is 10:00 a.m. to 7:00 p.m. CST. You will spend a significant portion of your day counseling patients by phone, reviewing therapies, documenting interventions, and collaborating closely with nurses, clinicians, and pharmacy technicians. The environment emphasizes evidence-based care, strict compliance with state and federal regulations, and high standards of quality and safety, including sterile technique and hazardous waste management. Standard professional attire is expected in the pharmacy setting.
