Job Summary
Review prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability. Provide information and advice regarding drug interactions, side effects, dosage, and proper medication storage.
Key Responsibilities
- Prospective patient profile review
- Medication order verification
- Identification and resolution of medication-related problems
- Participation in interdisciplinary rounds
- Patient education
- Clinical consultation for providers and staff
- Supports operational and distributive pharmacy functions as needed
- Promotes evidence-based practice, patient safety, regulatory compliance, and continuity of care
Qualifications
- Graduate of an accredited School of Pharmacy
- Thorough knowledge of drugs and their actions
- Knowledge of state and local regulations
- Hospital experience or residency preferred
- Licenses must be in good standing by the State Board of Pharmacy in state(s) of practice and/or possess multi-state licensure privileges as required by position

About the Role
The Adult Internal Medicine Focused Pharmacist provides direct pharmaceutical care to adult inpatient populations through a rotating day/evening schedule focused primarily on medical and surgical units. This role is embedded within patient care areas and collaborates closely with providers, nursing staff, and interdisciplinary teams to optimize medication therapy and support safe, effective patient care.
