PH RN (PHP/DPH Home Visiting)

Job Responsibilities

• Provides professional nursing care in accordance with nurse protocols and programmatic policies and procedures.
• Outreach, support, and education to the target population and their surrounding community to reduce disparities in birth outcomes.
• Provides community health and case management services to high-risk perinatal women, infants, and children primarily through home visitation.
• Provides patient education and refers to agencies that can assist in meeting social and/or health needs.
• Records pertinent observations and response to treatment.

Minimum Qualifications

• Associate’s degree/diploma in nursing from an accredited college
• Completion of a PHN Course within one year of hire
• Current Georgia Registered Professional Nurse license
• CPR certification
• Valid driver’s license and reliable transportation

Preferred Qualifications

• Completion of a bachelor’s degree in nursing
• Bilingual-Spanish
• Appropriate interventions for high-risk prenatal patients
• Appropriate interventions for high-risk newborns and infants
• Home visitation experience
• One-year Labor and delivery experience
• One-year experience working in a prenatal care clinic
• One-year experience working with mothers and babies
