Job Responsibilities
- Provides professional nursing care in accordance with nurse protocols and programmatic policies and procedures.
- Outreach, support, and education to the target population and their surrounding community to reduce disparities in birth outcomes.
- Provides community health and case management services to high-risk perinatal women, infants, and children primarily through home visitation.
- Provides patient education and refers to agencies that can assist in meeting social and/or health needs.
- Records pertinent observations and response to treatment.
Minimum Qualifications
- Associate’s degree/diploma in nursing from an accredited college
- Completion of a PHN Course within one year of hire
- Current Georgia Registered Professional Nurse license
- CPR certification
- Valid driver’s license and reliable transportation
Preferred Qualifications
- Completion of a bachelor’s degree in nursing
- Bilingual-Spanish
- Appropriate interventions for high-risk prenatal patients
- Appropriate interventions for high-risk newborns and infants
- Home visitation experience
- One-year Labor and delivery experience
- One-year experience working in a prenatal care clinic
- One-year experience working with mothers and babies
