About the Role

The Personal Support Coach-DSP ensures that individuals receive supports in an integrated, dignified, and respectful environment, enabling them to be active in their chosen communities. Supports are provided according to personal preferences and in compliance with agency policies, state, and federal regulations.

Responsibilities

  • Uphold the organization’s mission and person-centered philosophy by empowering individuals to live, work, and thrive as they desire.
  • Assist with personal care, including toileting, feeding, and hygiene.
  • Support daily living skills such as meal planning and preparation, shopping, grooming, job skills, and home maintenance.
  • Model and assist with good hygiene, appropriate dress, and proper social interaction in home, community, and work settings.
  • Schedule and transport individuals to community events, social activities, worksites, and appointments.
  • Serve as a liaison between individuals and the community, using resources to keep them engaged.
  • Complete daily documentation, including Caretracker data, medication logs, and behavioral charting.
  • Administer medications as needed and report concerns to nursing staff.
  • Provide financial guidance, including budgeting, banking, and related documentation.
  • Follow person-centered and no-restraint philosophies in alignment with the agency’s mission.
  • Assist in developing, guiding, and executing Person-Centered Plans (PCPs).
  • Review and follow all Behavioral Support Plans (BSPs).
  • Report instances of abuse or neglect immediately.
  • Complete Incident Reports and Nursing Call Forms within 24 hours for serious incidents.
  • Assist in training new team members.
  • Maintain all required certifications and training for the position.
  • Track time accurately using the timekeeping system.
  • Follow all department and agency policies and procedures respectfully.
  • Work with individuals or locations outside regular assignments, including cross-departmental support.
  • Perform other duties as assigned, including special projects and cross-program development.

Requirements

  • High School Diploma or equivalent (required).
  • Two years’ experience in a residential setting (preferred).
  • CNA/GNA Certification (preferred).
  • CPR and First Aid Certifications (preferred).
  • Access to reliable transportation and an active, valid driver’s license with no more than two points.
  • Proof of current automobile insurance.
  • Maintain all required trainings under DDA guidelines and agency policies.
  • Flexibility in scheduling to meet agency and program needs.

Physical Demands

The role requires physical ability to support and lift adults with developmental and other disabilities, including transferring individuals to and from wheelchairs, beds, toilet seats, showers, and vehicles. Employees may need to exert 50-70 pounds of force occasionally. The position involves:

  • Standing, walking, sitting, and using hands to handle objects or controls.
  • Climbing stairs, balancing, stooping, crouching, and kneeling.
  • Hearing (with or without devices), seeing (with or without corrective lenses), and speaking.
  • Working with individuals who may exhibit assertive or unpredictable behavior.
  • Physically intervening (when appropriate and approved) to prevent harm to individuals or others.
  • Safely evacuating individuals during emergencies.
