Responsibilities

- Provide 1-on-1 care to individuals in the Ulster County community. - Work in various settings as assigned. - Accept shifts based on availability and case contact.

Requirements

- Possess a valid New York State PCA Certificate. - Provide proof of current flu vaccination. - Provide proof of MMR vaccination, including reference ranges. - Have an annual physical from the past 365 days, stamped and signed by a physician. - Provide tuberculosis status, either through a Quantiferon blood draw within 9 months or two PPD tests, one within 30 days and one within 365 days, if positive, provide a chest X-ray from within 5 years or a doctor's note.
