Responsibilities

- Provide 1-on-1 care in the communities of Dutchess County. - Work per diem with individuals utilizing certified care professionals in various settings. - Have complete flexibility over accepted shifts and right of refusal for new cases.

Requirements

- Possess a valid NYS PCA Certificate. - Provide proof of current flu vaccination. - Provide proof of MMR vaccination, including reference ranges. - Provide an annual physical from the past 365 days, stamped and signed by a physician. - Provide tuberculosis status, either through a Quantiferon Blood Draw within 9 months or two PPD Tests, one within 30 days and one within 365 days. If positive, provide a chest X-ray from within 5 years or a doctor's note.
