Essential Functions
- Coordinate referrals, intake appointments, interviews, and housing lease-up activities with Property Management and DHS.
- Complete client intakes and facilitate housing placement within established program timelines.
- Develop individualized housing stability plans and connect clients to appropriate supportive services and community resources.
- Provide case management, crisis intervention, housing retention support, and ongoing client engagement.
- Aid clients with employment readiness, training opportunities, and job placement resources.
- Maintain accurate and timely client documentation in CHAMP and client files, ensuring all contacts and required documents are entered within 24 hours.
- Conduct regular follow-up with participants to support housing stability and program compliance.
- Collaborate with Property Management, DHS, community partners, and internal teams to ensure coordinated care.
- Attend case conferences, team meetings, and community events as required.
- Maintain compliance with HIPAA, 42 CFR Part 2, and all funding and program requirements.
- Represent the agency professionally and maintain appropriate professional boundaries.
- Perform other duties as assigned.
Minimum Qualifications
- Associate’s Degree in Social Services, Business, Housing, or a related field from an accredited institution; OR two (2) years of experience in social services, case management, homelessness services, or a related field.
- Knowledge of case management practices, crisis intervention, housing services, and community resources.
- Proficiency with Microsoft Office Suite and database systems.
- Strong written and verbal communication skills.
- Ability to work effectively with individuals from diverse cultural, ethnic, and socioeconomic backgrounds.
- Must successfully complete employment eligibility verification and background clearance requirements.
- TB clearance required prior to hire or within seven (7) days of employment and renewed annually.
- CPR and First Aid certification required within 30 days of employment.
- Valid California Driver’s License, reliable transportation, and current auto insurance required.
