Job Summary
Responsibilities
- Provide occupational therapy assessments and individualized treatment plans for clients.
- Deliver hands-on OT services to clients in onsite clinical and community settings.
- Monitor client progress and adjust plans to optimize functional outcomes.
- Collaborate with multidisciplinary team to coordinate patient-centered care.
- Adhere to regulatory, ethical, and clinical guidelines governing occupational therapy.
- Document assessments, treatment notes, and outcomes in electronic health records.
- Develop and implement evidence-based intervention plans aligned with each client’s goals.
- Track and report outcomes to meet quarterly targets and maintain high visibility of work.
- Engage in active pipeline and territory planning to identify potential referrals and caseload growth.
- Educate clients and families on activities of daily living and home programs.
- Identify opportunities for professional development and pursue certifications relevant to OT practice.
- Mentor junior staff and share best practices to foster a learning culture.
Requirements
- Licensed OT with active state registration and up-to-date credentials.
- Completion of mandatory OT fieldwork and hands-on therapy delivery experience.
- Demonstrated ability to develop and implement individualized OT treatment plans.

- Proficient documentation in electronic health records with outcome tracking.
