Talented Medical Solutions is seeking an Occupational Therapist (OT) for a full-time in-office position at a skilled nursing facility (SNF) in Williow, CA.

  • Deliver occupational therapy assessments and develop individualized treatment plans.
  • Implement evidence-based OT interventions to improve functional outcomes.
  • Monitor patient progress and adjust plans to meet goals.
  • Document assessments, treatments, and outcomes in electronic health records.
  • Meet or exceed productivity and outcome targets through pipelines and metrics.
  • Ensure compliance with regulatory and facility policies and ethical standards.
  • Collaborate with multidisciplinary team to ensure integrated care.
  • Engage in professional development to advance clinical skills and career growth.
  • Educate patients and families on home programs and activity strategies.
  • Contribute to clinical quality improvement initiatives and best practice sharing.
  • Provide mentorship or guidance to junior staff and students when applicable.
  • Participate in on-site community outreach to expand referral networks.

On-site work location in Williow, CA — collaborate in person with the team at our local office

Must be licensed/registered as an Occupational Therapist in the relevant state/country.

Minimum 2–5 years of occupational therapy practice, preferably in general or integrated care.

Ability to deliver assessments and develop individualized, evidence-based treatment plans.

Strong productivity mindset with ability to meet/track targets via pipelines and metrics.

Proficiency in electronic health records and documentation of assessments, treatments, and outcomes.

Excellent collaboration with multidisciplinary teams and clear communication.

Demonstrated commitment to regulatory compliance, privacy, ethics, and facility policies.

Commitment to professional development and career growth.

Ability to educate patients/families on home programs and activity strategies.

Mentorship capability and willingness to guide junior staff or students.
