PERM Director of Rehab (Physical Therapist Only)

Key Responsibilities

• Lead and oversee rehabilitation services within the facility
• Provide hands-on Physical Therapy treatment as needed
• Develop and maintain individualized treatment plans
• Supervise rehab operations and ensure compliance with all regulations
• Collaborate closely with nursing, physicians, and interdisciplinary teams
• Mentor and support rehab staff while promoting clinical excellence
• Ensure high standards of patient outcomes and satisfaction

Required Qualifications

• Licensed Physical Therapist (PT) in Pennsylvania
• Prior leadership or rehab management experience preferred
• Strong communication and organizational skills
• Passion for geriatric care and resident-centered therapy
• Experience in assisted living, SNF, or personal care settings is a plus
