Job Responsibilities

• Provides professional nursing care in accordance with nurse protocols, policies and/or procedures
• Focuses on the prevention of illness, injury, or disability, the promotion of health and maintenance of the health of populations
• Cares for populations with special needs during disasters/emergencies and biological and chemical protocols
• Identify high risk populations and tailor interventions based on the needs of the population and resources available
• Provide home visiting services to identify, assess and provide care coordination services to eligible perinatal patients and qualifying infants
• Carefully coordinate program services with other service providers and community organizations to promote optimal implementation
• Screen and refer clients to other community service providers
• Maintain monitoring of per program guidelines, educational instruction, materials to promote wellness, safety, and pregnancy and postpartum complications
• Maintain monitoring of infant complications to include feeding, weight, head circumference, and developmental screening
• Develop a plan of care for individuals and families, modify treatment plans as indicated by client’s responses and conditions
• Provide referral/linkage when medical and environmental risks are identified to appropriate services
• Promote program through community engagement and outreach events
• Develop and sustain community partnerships
• Must have the ability to work independently, be self-motivated and have exceptional time-management, interpersonal and organizational skills
• Maintain knowledge of and seek out new community resources
• Have reliable transportation
• Travel is reimbursed at the State of Georgia mileage rate
• Position will provide home health visits in Richmond, Emanuel, Jenkins, Burke, Screven and Jefferson County
• Home visits anticipated to expand through all district 6 counties
• All East Central Health District 6 employees are deemed essential workers in the event of an emergency
• As an employee of DPH, in the event of an identified emergency you may be required, as a term and condition of employment, to assist in meeting the emergency responsibilities of the department

Minimum Qualifications

• Bachelor’s degree in nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification, OR
• Associate degree in nursing from an accredited, college or university AND One year of nursing experience AND current Georgia Registered Professional Nurse license AND CPR certification

Preferred Qualification

• Perinatal and/or infant experience
• Bilingual
• Computer skills
• Outreach service experience
• MAPEP
