About the Role

The Performance Improvement Consultant is a mid-level professional individual contributor on the Strategic Implementation and Governance team, responsible for driving meaningful organizational change through performance improvement initiatives. This role focuses on planning, executing, and overseeing mid-sized performance improvement projects, delivering critical performance insights to CCC management, fostering a culture of continuous improvement, and supporting robust governance processes.

The first 12 to 18 months will heavily focus on behavioral health projects, including:

  • Leading the training and implementation of updated behavioral health clinical documentation and workflows to meet new state requirements.
  • Redesigning outpatient and outreach program models to improve client access, staff experience, and financial sustainability.

This role involves hands-on work spanning current-state mapping, designing better workflows, and ensuring adoption of changes. A key responsibility is partnering with leaders and stakeholders who supervise frontline staff to support change management and build buy-in.

Responsibilities

Performance Improvement Project Design and Leadership (60%)

  • Plan and implement assigned mid-sized performance improvement projects:
    • Lead problem definition using root cause analysis and problem-solving tools.
    • Define project goals, objectives, scope, and deliverables.
    • Structure workstreams and assess resource/capacity impact.
    • Collaborate with stakeholders to address resource considerations through governance.
  • Assemble and manage project teams, build consensus, and mentor team members.
  • Design and facilitate project kickoff sessions, work sessions, and other engagements.
  • Create, maintain, and utilize project management, change management, and performance improvement tools.
  • Monitor and control projects:
    • Lead plan-do-study-act cycles and other improvement methodologies.
    • Conduct data analysis using Lean/Six Sigma tools to assess improvement effectiveness.
    • Report on progress, risks, mitigation, and key decisions to stakeholders.
  • Develop project sustainment plans and ensure successful handoff to operational management.
  • Evaluate and assess project results and ensure project closure, documentation, and artifact completion.

Performance Analysis and Insights (20%)

  • Analyze performance trends on prioritized metrics (pay-for-performance, strategic outcomes, operational metrics) to identify improvement opportunities.
  • Produce planning and analytic work products using visualization and written descriptions.
  • Use analysis and visualization tools (e.g., Excel) to complete work.
  • Engage in thought partnership to inform annual/quarterly planning and governance decisions.
  • Analyze external quality metrics programs, including metric specifications and value-based reimbursement methodologies.
  • Support development of internal quality metrics in partnership with Decision Support.
  • Advise management on metrics, performance goals, and revenue contract optimization.
  • Assist with pay-for-performance revenue forecasts for annual budgets and midyear updates.

Governance and Planning (10%)

  • Advise management on identifying, developing, and prioritizing performance improvement projects aligned with organizational goals.
  • Participate in intake, review, and evaluation of project proposals for strategic fit, feasibility, and impact.
  • Support resource planning by identifying staffing, data, and other requirements.

Training and Consultation (10%)

  • Participate in developing performance improvement and project management tools/processes.
  • Contribute to learning and development materials for performance improvement and project management.
  • Train CCC leaders and staff to participate in and lead performance improvement projects.
  • Provide coaching and guidance during performance improvement and project management consultations.

Requirements

Minimum qualifications (one of the following):

  • Master’s degree in social work, public health, public administration, business, or a relevant field and 3 years of quality or project management experience.
  • Bachelor’s degree in a relevant field and a professional certification (e.g., Lean/Six Sigma Green Belt, human-centered design) and 3 years of quality or project management experience.
  • Bachelor’s degree in a relevant field and 5 years of quality or project management experience.

Required experience: Quality or project management in a healthcare, social services, or safety net setting.

Technical skills:

  • High computer literacy, including ability to learn new applications.
  • Proficiency in Microsoft Office and workflow diagramming tools (e.g., Lucid, Visio).
  • Strongly preferred: Experience with electronic medical records, case management software, or web-based collaboration tools (e.g., Mural, Miro).

Additional requirements:

  • Pass pre-employment drug screen, TB test, and background check.
  • Physical ability to bend, stoop, kneel, lift heavy objects, climb stairs, and carry equipment between locations.
  • Adhere to agency’s non-discrimination policies.
  • Ability to interact effectively with diverse coworkers and clients, respecting individual differences.

Skills and Abilities

  • Practical project management knowledge, including the Project Management Life Cycle.
  • Demonstrated skills in project identification, planning, execution, control, and assessment.
  • Strong quality assessment, problem analysis, and performance improvement skills (e.g., Lean, Six Sigma, Model for Improvement).
  • Analytic skills, including data assembly, statistical analysis, and visualization.
  • Ability to apply project management techniques and tools.
  • Ability to design and facilitate in-person, virtual, and hybrid work sessions.
  • Business acumen related to strategy, planning, performance improvement, and operations.
  • Ability to consider impacts on underserved communities, systems of oppression, and structural racism in decision-making.
  • Strong critical thinking, decision-making, and professional judgment.
  • Leadership skills, including coaching and stakeholder management.
  • Ability to manage time, meet deadlines, and work independently or collaboratively.

Pay

$87,000 - $113,000 annually. Final compensation determined based on relevant experience, education, credentials/licensure, and internal equity.

Schedule

Monday - Friday, 8:00 AM to 5:00 PM.
