Care Manager – Client Assessments & Non-Medical Care Oversight About Us: CareBuilders at Home is dedicated to helping individuals live safely, comfortably, and with dignity in their own homes. As we grow in Connecticut, we are seeking compassionate Registered Nurses to support client assessments, care planning, and caregiver guidance on a flexible, PRN basis. Job Description for a Care Manager: This role is ideal for an RN who enjoys supporting families, guiding caregivers, and building trust with clients, while maintaining a flexible and predictable workload. You will play an important role in ensuring our clients receive safe, high-quality, and person-centered care, without the demands of shift nursing or full caseload management. Responsibilities of a Care Manager: Complete initial client assessments and periodic reassessments Develop and update individualized care plans tailored to client needs, preferences, and safety Conduct supervisory visits to support caregivers and ensure high-quality care Provide coaching, education, and encouragement to caregivers Communicate with families to offer updates, guidance, and reassurance Monitor client progress and recommend care adjustments when needed Document assessments, care plans, and visit notes in the agency's system Participate in occasional care conferences or team discussions to support continuous improvement Support compliance with Connecticut home care standards and agency policies Coordinate and oversee the implementation of care plans Provide ongoing monitoring and evaluation of client progress and adjust care plans as necessary.
