About the role

Per Diem (PRN) Registered Nurse providing adult and geriatric home care services through a Licensed Home Care Services Agency (LHCSA) in Suffolk County, NY.

Responsibilities

• Conduct in-home assessments and create individualized care plans
• Provide clinical oversight, coaching, and support to caregivers and field nurses
• Ensure compliance with LHCSA and NYS DOH standards through accurate, timely documentation
• Collaborate closely with families, providers, and interdisciplinary team members
• Support staff education and quality improvement initiatives

Requirements

• Active, unrestricted RN license in New York
• BSN preferred
• Minimum 2 years of nursing experience, including at least 1 year in home care
• Geriatrics experience preferred
• Strong clinical assessment, organizational, and leadership skills
• Excellent communication and time-management abilities
• Dependable transportation for local travel within Suffolk County
