About the Role

The Health Center Nurse (LPN/RN) is an integral member of the primary care team, providing compassionate, high-quality, patient-centered care to individuals across diverse patient populations. This role reports to the Practice Manager and works under the daily direction and clinical oversight of the provider staff.

Responsibilities

  • Patient Care
    • Coordinate with office staff to monitor and adjust scheduling and workload of providers.
    • Interview patients for health history and clinical information updates.
    • Contribute to patient assessments through data collection using established protocols.
    • Take patient vital signs (temperature, pulse, respiration, blood pressure, etc.).
    • Apply and change dressings/bandages.
    • Administer IM, SQ, intradermal, inhaled medications, and vaccines.
    • Administer allergy injections per site protocols.
    • Perform catheterization per provider orders and within scope of protocols.
    • Conduct vision and audiometry screenings.
    • Perform EKGs (if available on site).
    • Assist in suturing procedures.
    • Start IVs and respond to Code Blue/defibrillation needs as per protocols.
  • Health Education/Teaching
    • Instruct patients about treatment regimens (medication, diet, activity, etc.).
    • Provide education on health maintenance.
    • Participate in community health education initiatives (e.g., screening clinics, public forums).
  • Laboratory Testing
    • Perform laboratory tests as per health center protocols.
    • Record tests and results in the appropriate EMR.
    • Draw and prepare specimens for in-house or outside laboratory testing (phlebotomy).
  • Pharmaceuticals
    • Dispense pharmaceuticals per provider orders and in accordance with health center, NCHC, Vermont, and Federal guidelines.
    • Monitor inventory levels and expiration dates of on-site pharmaceutical supplies.
  • Records and Documentation
    • Document nursing activities and patient care information in medical records per protocols.
    • Maintain records and statistics (e.g., immunizations, perinatal data) as required.
  • Infection Control
    • Clean and sterilize instruments.
    • Maintain general office infection control.
  • Quality Assurance
    • Participate in the development, implementation, audit, and continuous improvement of protocols, services, and programs.
  • Meet productivity standards set by the Supervisor and/or NCHC Division protocols.
  • Perform other job-related tasks as requested.

Requirements

Candidates must hold either a Licensed Practical Nurse (LPN) or Registered Nurse (RN) license. Responsibilities may be adjusted based on licensure, experience, and scope of practice.

This position has no direct supervisory responsibilities.
