Per Diem Continuous Care LPN

Job Summary

Responsibilities

• Provides continuous bed-side clinical observations, prescribed care, physical, and emotional support to patients as directed in plan of care, reporting significant changes in patient condition and family needs as needed.
• In collaboration with the RN, accepts and processes physician orders to maximize patient comfort.

Qualifications

• Graduate of an accredited practical nursing program.
• Active Licensed Practical Nurse (LPN) in the State of New Jersey.
• Minimum of 1-2 years of clinical experience, preferably in hospice, long-term care, home health, or acute care.
• Understanding of Hospice philosophy and needs of terminally ill.
• Strong problem solving and critical thinking skills.
• Ability to travel within the state of NJ.
