About the role
Continuing Care Peer at Community Bridges, Inc. (CBI) offers community-based outreach, support, navigation, and care coordination for individuals experiencing behavioral health challenges, substance use, homelessness, or crisis situations.
Responsibilities
- Provide peer support, outreach, care planning, and resource linkage both onsite and in the community.
- Support individuals during deflection encounters, sobering services, and transitions to appropriate levels of care.
- Ensure follow-up, advocacy, and continuity of services.
Requirements
- High school diploma or GED required.
- Minimum six months of recovery from substance use and/or mental health challenges.
- Experience in outreach, peer support, homeless services, or behavioral health preferred.
- Driver’s license (valid and in good standing) required.
- Clear 39-month motor vehicle record required.
- Peer Support Specialist certification from the Oregon Health Authority required or able to be obtained within 90 days of hire.
Skills
- Lived experience in recovery from substance use and/or mental health challenges.

Pay
$19/HR - $22.80/HR
Schedule
Hourly rate varies based on experience and qualifications.
