Responsibilities

  • Engage participants in transition planning and needs assessment
  • Conduct individual sessions focusing on independent living and coping skills
  • Provide emotional and practical support during community transition
  • Foster and maintain collaborative communication with community providers and collaterals
  • Advocate for participants' personal goals and address barrier to obtaining those goals
  • Maintain comprehensive documentation following contact with clients and collaterals

Qualifications

  • A High School Diploma or equivalent
  • New York State Certified Peer Specialist required
  • Experience in advocacy and behavioral health services preferred
  • Must have lived experience with mental health
  • Ability to use personal story for empowerment and recovery
  • The ideal candidate is a self-starter who can work independently as well as within a team and has excellent follow up skills

$45,000

Schedule

Hybrid schedule offered, with inpatient, home and community visits required.
