Responsibilities
- Provides peer support services either in office or in community for an assigned group of clients
- Maintains the activities of Care Coordination for high utilizing clients of New Horizons services
- Affords assistance to the Care Coordination Team with activities for assigned clients
- Prepares written documentation for each service conducted
- Provides transportation to consumers as needed
Requirements
- Certified as a Peer Specialist or in process of Certification preferred
- Valid and current Florida Driver's License required with no more than 6 points in the past 6 years
- Passes Level 2 Background through DCF and AHCA Clearinghouse
