Position Summary

As a Peer Navigator in Modesto, California, you will be assisting clients and their families in gaining access to primary preventative and specialty health care services. You will be providing specific assistance with medication management, housing, counseling, support and advocacy. Additionally, responsible for engaging, educating, and offering support to individuals and their family in order to successfully connect them to culturally relevant health services including prevention, diagnosis, timely treatment, recovery management, and follow-up.

Key Responsibilities

• Provides emotional support to alleviate fears of and barriers to accessing quality health care.
• Assists assigned clients to identify and progress toward achievement of their chosen goals.
• Motivates and educates individuals/families/caregivers about the importance of preventative services, medical conditions and recovery strategies.
• Advocates for clients in all areas of treatment and helps them apply for and receive services.
• Empowers clients in meeting their goals.
• Provides face-to-face contact with each client assigned.
• Advocates for and assists clients and their families in accessing and receiving mental health treatment, medical care, housing, food and clothing as indicated.
• Develops individual service plans for each client with their input whenever possible.
• Meets regularly with clients at Aspiranet sites, jobsites, homes, board and care facilities, skilled nursing facilities, psychiatric facilities and other appropriate sites.
• Arranges for or transports clients to and from medical appointments.
• Provides prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis situation.
• Completes paperwork as assigned in a timely manner.

Qualifications

Minimum Requirements:

• Bachelor's Degree in a social service/mental health related field and two years' experience in that field
• Knowledge of psycho-social rehabilitation's treatment and programming
• Ability to work and communicate with staff, clients, families, community agencies and professionals
• Ability to perform crisis intervention strategies
• Ability to work effectively under stress and conflict
• Ability to exercise appropriate judgment and decision-making
• Ability to be flexible and adaptable in any given situation
• Ability to work as a member of a team
• Ability to work under time deadlines and pressures
• Ability to work with minimal supervision
• Meets all state-required employment conditions, including:
  • DOJ fingerprint and Child Abuse Index clearance
  • TB/Health physical
  • Valid CA Driver's License, acceptable driving record, and proof of insurance

Preferred:

• Bi-lingual in County Threshold language(s)
• "Lived" experience is a plus

Work Environment & Schedule

• Field Based position that requires traveling to client's home or community setting
• Schedule is Monday-Friday Day Shift, with flexibility to work some evenings

Pay

The pay range for this position is $22.66 - $23.16 per hour for employees hired in the Central Valley, CA. The pay range provided in this job posting represents the typical range or starting rate of candidates hired within the Central Valley region. Candidates hired in Southern California or Bay Area locations may have higher starting salaries due to cost of living and market data differences. Factors that may be used to determine your actual starting salary may include your education, experience, knowledge, skills, abilities, the market data for your work location, and internal comparison to other employees who hold a similar role.
