Position Summary
The Peer Mentor for CHAT provides peer support to adolescent and young adult patients to help them gain self-advocacy and self-efficacy skills as well as skills to manage their health care needs. Facilitate and support access to CHLA and community resources to patients and families. Serve as a facilitator between patients, families, and their representatives across the enterprise. CHAT is accountable for implementing an institution-wide system for healthcare transition while patients are at CHLA and for transfer of care from CHLA to adult providers.
Schedule
Per-diem, 4pm on Tuesdays; mornings on Saturdays. Other hours flexibly scheduled per department needs.
Qualifications
- Experience as a patient in a children's hospital setting.
- Bilingual language capacity (Spanish) preferred.
- High school diploma or equivalent required.
