Role: Home and Community Based Services Licensed Practical Nurse (LPN) Responsibilities Include Providing direct nursing care to acutely and chronically ill patients in their homes, following physician prescriptions.Documenting patient visits thoroughly in electronic medical records and electronic visit verification systems.Participating in care coordination by reporting patient needs and changes, attending case conferences, and communicating effectively with patients, families, physicians, and care teams. Qualifications Current and unrestricted LPN licensure in the practicing state.CPR certification and a valid driver’s license with reliable transportation.Ability to serve diverse home environments and work independently.At least one year of LPN experience preferred. Skills: Strong communication, organizational abilities, multitasking, and adaptability to various home settings.
