About the Role

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely or chronically ill patients in their homes, following physician prescriptions.

Responsibilities

• Deliver high-quality clinical services within legal scope and plan of care, ensuring infection control protocols are followed.
• Thoroughly document patient visits and progress in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.

Requirements

• Current and unrestricted LPN licensure in the practice state.
• CPR certification, valid driver’s license, and access to reliable transportation.
• Ability to work in diverse home environments regardless of client background.

Qualifications

• At least one year of LPN experience is preferred.
• Home care experience is a plus.
