Pediatric Transitional Nurse (RN/LPN)

About the role

We are seeking Pediatric Transitional Nurses (RN or LPN) to support medically fragile children and their families during critical transitions to home care. In this role, you'll provide one-on-one pediatric nursing care in patients' homes, perform assessments, administer medications, support hospital-to-home discharges, educate families, and collaborate with interdisciplinary teams.

Responsibilities

• Provide one-on-one pediatric nursing care in patients' homes
• Perform assessments and administer medications
• Support hospital-to-home discharges
• Educate families on care management
• Collaborate with interdisciplinary teams
• Provide discharge support and interim coverage
• Train other nurses as needed

Requirements

• Active RN or LPN license
• Current CPR certification
• Experience with high-acuity pediatric patients
• Experience with trach, vent, and G-tube management
• Ability to lift pediatric patients
• Excellent emergency response skills

Qualifications

• Pediatric home health experience preferred
