Pediatric Transitional Nurse – Registered Nurse (RN) Licensed Practical Nurse (LPN)

About the role

Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Practical Nurses (LPN) for the Pediatric Transitional Nurse Team to support medically fragile children during critical transitions to home care. This role offers meaningful one‑on‑one pediatric care, flexible scheduling with on‑call support, a clinically led nurse‑founded organization, a supportive team culture with a strong mission, and opportunities to train and mentor other nurses. You will work with high‑acuity patients skilled in trach, vent, G‑tube, and emergency management, ensuring safe discharges, preventing readmissions, providing interim coverage, and supporting continuity of care until permanent staffing is established.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes
• Perform patient assessments and ongoing evaluations
• Administer medications, treatments, and therapies
• Support hospital‑to‑home transitions for high‑acuity patients
• Educate and support parents, families, and caregivers
• Assist with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Provide first‑shift and interim coverage as needed
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately in emergency situations and changes in patient condition

Qualifications

• Active, unrestricted Registered Nurse (RN) or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license (PA)
• Current CPR certification
• Ability to lift and assist pediatric patients as needed
• Pediatric home health experience
• Experience with Trach care and Ventilator management
• High‑acuity pediatric patient experience
• Preferred: TPN experience

Pay

$38 per hour (biweekly pay)

Schedule

• Full‑time position with guaranteed hours
• Day and night shifts available
• On‑call and staffing coverage responsibilities as needed
• Field‑based role with local travel
