Pediatric Transitional Nurse – Registered Nurse (RN) Licensed Practical Nurse (LPN)
About the role
Located in Harrisburg, PA, Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Practical Nurses (LPN) for the Pediatric Transitional Nurse Team to support medically fragile children and their families during critical transitions to home care. This role is ideal for nurses confident working with high‑acuity patients and skilled in trach, vent, G‑tube, and emergency management. As a Transition Nurse, you'll ensure safe, successful discharges, prevent readmissions, provide interim patient coverage, and support continuity of care until permanent staffing is established. Responsibilities
- Provide skilled pediatric nursing 1:1 care in patients’ homes
- Perform patient assessments and ongoing evaluations
- Administer medications, treatments, and therapies
- Support hospital‑to‑home transitions for high‑acuity patients
- Educate and support parents, families, and caregivers
- Assist with discharge support and care coordination
- Train and support permanent field nurses during transitions
- Provide first‑shift and interim coverage as needed
- Collaborate with Clinical Care Managers and interdisciplinary teams
- Respond appropriately in emergency situations and changes in patient condition

Qualifications
- Active, unrestricted RN or LVN/LPN license (PA)
- Current CPR certification
- Ability to lift and assist pediatric patients as needed
- Pediatric home health experience
- Experience with trach care and ventilator management
- Experience with high‑acuity pediatric patients
- TPN experience (preferred)

Pay
Pay Range: $40/hr (Biweekly pay)
Schedule
Full‑time position with guaranteed hours; Day and Night shifts available; On‑call and staffing coverage responsibilities as needed; Field‑based role with local travel.
