This role is ideal for nurses who are confident working with high-acuity patients and skilled in trach, vent, G-tube, and emergency management. As a Transition Nurse, you'll play a critical role ensuring safe, successful discharges, preventing readmissions, providing interim patient coverage, and supporting continuity of care until permanent staffing is established.
