Pediatric Transitional Nurse – Registered Nurse (RN) / Licensed Practical Nurses (LPN)

About the Role

Located in Charlotte, NC, Angels of Care Pediatric Home Health is seeking Registered Nurses (RN) or Licensed Practical Nurses (LPN) to join the Pediatric Home Team (Transitional Care Team). The position supports medically fragile children and their families during critical transitions from hospital to home. The role is ideal for nurses confident working with high‑acuity patients and skilled in tracheostomy, ventilator, G‑tube, and emergency management.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes
• Perform patient assessments and ongoing evaluations
• Administer medications, treatments, and therapies
• Support hospital‑to‑home transitions for high‑acuity patients
• Educate and support parents, families, and caregivers
• Assist with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Provide first‑shift and interim coverage as needed
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately in emergency situations and changes in patient condition

Qualifications

• Active, unrestricted RN or LPN license (NC)
• Current CPR certification
• Ability to lift and assist pediatric patients as needed
• Emergency response management skills
• Preferred: Pediatric home health experience
• Experience with tracheostomy care, ventilator management, and TPN
• Experience caring for high‑acuity pediatric patients
• Care coordination, discharge planning, or transitional care experience

Pay

Weekly pay; $34‑$40 per hour.

Schedule

• Full‑time position with guaranteed hours
• Day and night shifts available
• On‑call and staffing coverage responsibilities as needed
• Field‑based role with local travel
