Pediatric Transitional Nurse – Registered Nurse (RN) / Licensed Vocational Nurse (LVN)

About the role

Angels of Care Pediatric Home Health is seeking RNs and LPNs to join our Pediatric Transitional Nurse Team. This role is perfect for nurses who work confidently with high-acuity patients and have experience with trach, vent, G-tube, and emergency management.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients' homes
• Perform patient assessments and ongoing evaluations
• Administer medications, treatments, and therapies
• Support hospital-to-home transitions for high-acuity patients
• Educate and support parents, families, and caregivers
• Aid with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately in emergency situations and changes in patient condition

Requirements

• Active Texas Registered Nurse or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license
• Current CPR certification
• Ability to lift and assist pediatric patients as needed
• Experience with Trach care and Ventilator management
• High-acuity pediatric patients (preferred)
• Emergency response management
• Pediatric home health experience (preferred)

Qualifications

• High-acuity pediatric patients
• Full-time position, with guaranteed hours
• Flexible scheduling based on patient needs and Nurse availability
• 5 working days per week, minimum 1 weekend day
• On call and Staffing coverage responsibilities as needed
• Local travel within approximately 30–40 miles
