About the role
Angels of Care Pediatric Home Health is seeking RNs and LPNs to join our Pediatric Transitional Nurse Team. This role involves providing skilled pediatric nursing 1:1 care in patients' homes, performing patient assessments and ongoing evaluations, administering medications, treatments, and therapies, supporting hospital-to-home transitions, educating and supporting parents, families, and caregivers, assisting with discharge support and care coordination, training and supporting permanent field nurses during transitions, collaborating with Clinical Care Managers and interdisciplinary teams, responding appropriately in emergency situations and changes in patient condition.
Responsibilities
- Provide skilled pediatric nursing 1:1 care in patients’ homes
- Perform patient assessments and ongoing evaluations
- Administer medications, treatments, and therapies
- Support hospital-to-home transitions for high-acuity patients
- Educate and support parents, families, and caregivers
- Aid in discharge support and care coordination
- Train and support permanent field nurses during transitions
- Collaborate with Clinical Care Managers and interdisciplinary teams
- Respond appropriately in emergency situations and changes in patient condition
Requirements
- Active Texas Registered Nurse or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license
- Current CPR certification
- Ability to lift and assist pediatric patients as needed
- At least 3 years of experience with Trach care, Ventilator management, and TPN, post-graduation
- Home Health Care environment experience
- Emergency response management
- High-acuity pediatric patients (preferred)
Qualifications

Pay
- RN: $35–$42/hour
- LVPN: $30–$35/hour
Schedule & Travel
- 12 hour shifts (Day or Night)
- Flexible scheduling based on patient needs and Nurse availability
- 5 working days per week, minimum 1 weekend day
- On call and Staffing coverage responsibilities as needed
- Local travel within approximately 30–40 miles
