Pediatric Transitional Nurse – Registered Nurse (RN) / Licensed Vocational Nurse (LVN)

About the role

Angels of Care Pediatric Home Health is seeking RNs and LPNs to join our Pediatric Transitional Nurse Team. This role is designed for nurses who are comfortable with high-acuity patients and specialized care such as tracheostomy, ventilator management, and gastrostomy tube care.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients' homes
• Perform patient assessments and ongoing evaluations
• Administer medications, treatments, and therapies
• Support hospital-to-home transitions for high-acuity patients
• Educate and support parents, families, and caregivers
• Aid with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately in emergency situations and changes in patient condition

Qualifications

• Active Texas Registered Nurse or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license
• CPR certification
• At least 3 years of experience with Trach care, Ventilator management, and TPN, post-graduation
• Home Health Care environment experience
• High-acuity pediatric patients Emergency response management

Schedule & Travel

• 12 hour shifts (Days or Nights)
• Flexible scheduling based on patient needs and Nurse availability
• 5 working days per week, minimum 1 weekend day
• On call and Staffing coverage responsibilities as needed
• Local travel within approximately 30–40 miles
