Pediatric Transitional Nurse – Registered Nurse (RN)

About the role

Angels of Care Pediatric Home Health is hiring Registered Nurses (RN) and Licensed Vocational Nurses (LVN) for our HALOS Home Team to support medically fragile children and their families during critical transitions to home care. This role is ideal for nurses who are confident working with high-acuity patients and skilled in trach, vent, G-tube, and emergency management.

Responsibilities

• Provide skilled pediatric nursing 1:1 care in patients’ homes
• Perform patient assessments and ongoing evaluations
• - Administer medications, treatments, and therapies
• Support hospital-to-home transitions for high-acuity patients
• Educate and support parents, families, and caregivers
• Assist with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately in emergency situations and changes in patient condition

Requirements

• Active Georgia Registered Nurse (RN) or Licensed Vocational Nurse (LVN) license
• Current CPR certification
• Ability to lift and assist pediatric patients as needed
• Experience with Trach care and Ventilator management
• High-acuity pediatric patients
• Emergency response management

Qualifications

• Pediatric home health experience
• Full-time position, with guaranteed hours
• Flexible scheduling based on patient needs and Nurse availability
• 5 working days per week, 1 weekend day strongly preferred
• On call and Staffing coverage responsibilities as needed
• Local travel within approximately 30–40 miles
